Application for entry on Québec’s permanent list of electors

1. One identity document must show your name and date of birth. It could be your:
   • Canadian passport
   • Health insurance card
   • Citizenship certificate
   • Birth certificate
   • Certificate of Indian Status
   • Canadian Armed Forces identification card
2. The other identity document must show your name and current domiciliary address in Québec. It could be:
   • Your driver’s licence
   • A hospital card
   • Your lease
   • An account statement
   • School report card

The information you provide on this form is required to register to vote outside Québec. All information collected is mandatory, unless otherwise indicated.

This information is confidential under the Act respecting access to documents held by public bodies and the protection of personal information. It will only be used for the purposes set out in the Election Act. Élections Québec retains this information only as long as it is required to fulfill the purposes set out in the Act, subject to the rules set out in its retention schedule. The institution takes the necessary security measures to ensure the protection of personal information that it collects, uses, communicates, retains or destroys.
